Item Adaptation to the worker role: The vocational experience of South African male ex offenders(OMICS International, 2014) Soeker, Mogammad ShaheedObjectives: The study is aimed at exploring, describing and understanding the experiences and perceptions of male ex-offenders with regard to adapting to their worker roles after they have been released from prison. Methods: The research methodology utilised was positioned in the qualitative research paradigm, specifically using the tradition of interpretivism. To gather data from the participants, focus groups and face to face interviews were conducted, consisting of broad questions followed by probes to gain an in-depth description of participants’ experiences and perceptions. Participants: Five males who were classified as being ex-offenders participated in the focus groups and one female (key informant) participated in two semi structured interviews. Results: Four themes emerged that reflected the vocational experience of male ex- offenders: 1) A sense of distrust, 2) A sense of support: Giving back to the community through learning and doing, 3) A change in lifestyle and 4) Working towards change through capacity building. For the purpose of this article, Theme 3- A change in lifestyle, will be discussed in depth as it explored the adaptive process that male ex- offenders experienced when adapting to their worker roles post release from prison. Conclusions: The findings of the study suggest that occupational therapy practitioners as well as correctional service personnel should utilise context specific vocational rehabilitation programmes that enhance the exoffender’s ability to find employment once they have been released from prison. Practitioners should be aware of both the internal and external adaptation strategies when planning vocational rehabilitation and community reintegration strategies.Item A descriptive, qualitative study of the challenges that individuals with Autism Spectrum Disorder experience when transitioning from skills training programs into the open labor market in Cape Town, South Africa(IOS Press, 2020) Soeker, Mogammad ShaheedBACKGROUND: There is limited existence of qualitative research on adults with Autism Spectrum Disorder (ASD), who are transitioning between skills training programs and finding employment in the open labor market, in South Africa. OBJECTIVE: The aim of this current study was to explore the experiences and perceptions of individuals living with ASD, regarding transitioning from a skills training program to working in the open labor market. PARTICIPANTS: The data were collected through semi-structured interviews with ten participants, who were diagnosed with ASD, and working in the open labor market, as well as five key informants, who have worked with autistic individuals in a skills training program. METHODS: The data collected, using an explorative, descriptive, qualitative method, were analyzed using thematic analysis, and resulted in three emerging themes. RESULTS: Theme one: The 'disorder' in the workplace -The individual and collective barriers that individuals experience in the workplace are described, including social, emotional, and communication difficulties, as well as the stigmatization experienced by persons with a disability. Theme two: Making the workplace work -The facilitative support that individuals are exposed to, in the form of intervention from occupational therapists, sensory psychologists, and other forms of skills training programs, are described. In addition to supportive intervention, the relationships and networking within, and outside of the workplace, are discussed in theme two. Theme three: What works for me -The individual adaptations made by the participants, in the form of unique coping mechanisms in the workplace, as well as in the home environment, are discussed. CONCLUSION: It was observed that, while there are barriers to the transitioning of autistic individuals into the open labor market, there are enablers that facilitate a less challenging transitioning process, in the form of family and workplace support, as well as skills training, and self-identified coping mechanisms. © 2020 - IOS Press and the authors. As a result, multi-drug-resistant TB (MDR-TB) has become a serious public health issue. Underpinning this study was the assumption that a client-centred approach to treatment of MDR-TB clients, with a hospital programme which adopts an integrated multidisciplinary approach that is client-centred and is not purely biomedically driven, would improve treatment outcomes of MDR-TB clients.Item The experiences of individuals with schizophrenia using The model of Occupational Self-Efficacy in enhancing work skills and returning to work in the open labour market in Western Cape, South Africa(South African Journal of Occupational Therapy, 2021) Soeker, Mogammad Shaheed; Abbas, IesrafeelIntroduction: Evidence suggests that supported employment strategies improve the rates of employment for individuals with schizophrenia. The Model of Occupational Self-Efficacy (MOOSE) is such a strategy aimed at enhancing work skills and facilitating return to work (RTW). The aim of the current study was to explore the experiences of individuals with schizophrenia about the use of the MOOSE in enhancing work skills and facilitating RTW in the open labour market. Method: A qualitative research project with an interpretivist paradigm explored the experiences and perceptions of nine participants diagnosed with Schizophrenia. Two occupational therapists participated as key informants. Data were collected by means of semi-structured interviews and analysed using inductive thematic analysis. Results: Barriers and facilitators to the use of the MOOSE were revealed. More importantly, the MOOSE enhanced the development of worker identities and contributed to the successful adaptation of worker roles in the RTW process of individuals with schizophrenia in the open labour market.Conclusion: The MOOSE is an effective supported employment strategy for persons with schizophrenia. Factors such as client-centred practice, the presence of social support, and ongoing therapeutic support contributed to the participants’ successful RTW experience. In addition, the Code of Good Practice as described in South African Labour Law serves as a guide for employers to encourage equal and fair opportunities for employees with disabilities. However, despite the presence of this policy, as well as other legislation, research has shown that individuals with TBI are failing to return to work. The Model of Occupational Self-Efficacy (MoOSE) was developed as a client centered approach to return individuals with TBI to work. The aim of the current study was to explore, and describe the experiences and perceptions of occupational therapists who have used the MoOSE in the vocational rehabilitation and return to work process of clients with TBI. The participants in the study were selected by means of purposive sampling and semi-structured face-to-face interviews were conducted on a monthly basis for three months with 10 occupational therapists. The semi-structured interviews were used to gain insight into and explore the perceptions of the occupational therapists who have used the MoOSE in their vocational rehabilitation programmes with clients who have suffered TBI. The findings of the study were analyzed by means of thematic content analysis. The outcome of this study was aimed at finding ways to enhance / improve the model when it is used in the vocational rehabilitation process with people who have suffered TBI. Informed consent was obtained from the research participants and confidentiality of their information was maintained. Data was collected by means of semi-structured interviews which were audiotaped and transcribed. Transcriptions were analysed by using constant comparative methods of data analysis which resulted in categories, which reflected the purpose of the study being created. The analysis of transcripts was done using Microsoft Word. Results were then divided into themes and discussed holistically. Four themes emerged that answered the research question. Theme one described how client related factors impacted on the implementation of the MoOSE. Theme two explored the therapist related factors that impacted on the implementation of the MoOSE while theme three discussed how characteristic of the model impacted on the vocational rehabilitation and return to work process. The participants appreciated the clear four stages of the MoOSE and the dynamicity of the model. Theme four described how the job market and employer perceptions impacted on the return to work process. Recommendations were then discussed that could further improve the model and the implementation thereof. Recommendations included adding a motivation component to the MoOSE, involving family members in the rehabilitation process and therapist reflection as a means to foster client-centered practice. On 15 March 2020, the president of South Africa, Cyril Ramaphosa, declared a national state of disaster.Item An exploratory, phenomenological study of the maternal-intergenerational transference of sexual abuse(University of the Western Cape, 2015) Adams, Cecille Arlene; Soeker, Mogammad ShaheedChildhood sexual abuse transcends boundaries of social class, race, age and gender. Research suggests that its effects are so profoundly debilitating that even in adulthood, victims struggle with a range of emotional, psychological and behavioural challenges. Nationally, South Africa has experienced a high prevalence of childhood sexual abuse, with a noticeable reporting of sexual abuse amongst children whose mothers survived childhood sexual abuse. The current study was aimed at exploring and describing the experiences of maternal survivors of childhood sexual abuse (CSA) whose children also disclosed childhood sexual abuse. It furthermore explored the maternal intergenerational transference of sexual abuse and its influence on parenting, as well as exploring how the experience of childhood sexual abuse influences maternal parental practice and confirms or negates whether sexual abuse is generational. A qualitative research design was used to explore the experiences and perceptions of seven mothers who experienced sexual abuse as children, whose children also disclosed sexual abuse. The sample was purposefully drawn and the interviews were conducted at the premises of two counselling organisations, where they (mothers/children) were receiving counselling. The researcher made use of in-depth interviewing with maternal survivors of childhood sexual abuse whose children also disclosed childhood sexual abuse. The data was collected using an interview schedule with open-ended questions, which facilitated the interview process. Interviews that were conducted with the maternal survivors of childhood sexual abuse were recorded and transcribed verbatim. The data from the study was analysed using thematic analysis, with all data managed manually. Ethical considerations were deliberated to participants who provided signed, informed consent for participation in the study. The findings were revealed in four themes. Theme one describes the experience of loss as a result of the CSA, which included loss of childhood, loss of relationships with family and friends, and loss of parental attachment. Theme two describes the interpersonal and psychological challenges resulting from CSA. A range of psychological and interpersonal sequelae resulting from the CSA experience poses many challenges for the maternal survivor of CSA. These include post-traumatic stress, cognitive distortions, emotional distress, avoidance, and interpersonal difficulties. Theme three describes the barriers affecting the maternal survivor's sense of Self, which includes negative body image and negative self-image. Maternal survivors of CSA struggle with issues pertaining to negative body image, which is as a result of the CSA experience. Issues pertaining to negative self-image were also captured. Theme four describes the parental practice of the maternal survivor and coping capacity, which includes challenges affecting the parental role, the maternal survivor's reaction to her children’s disclosure, as well as the coping strategies adopted to cope and move on. The results of the study show that maternal survivors of CSA experienced a shared feeling of loss, both on a personal level and a relational level. The findings furthermore indicate that the interpersonal and psychological challenges experienced by the maternal survivor are a direct consequence of the CSA and adversely affect the parental role of the maternal survivor. The findings indicate that in cases of intra-familial CSA, the chances are increased for CSA to re-occur in subsequent generations when contact with the mother's perpetrator is maintained. However for the purpose of this paper there will be a focus only on the barriers and facilitators that influence the return to work process. Participants: Ten individuals who were diagnosed with a mild to moderate brain injury participated in this study. Methods: Qualitative research methods were used in order to explore the research question. The participants were selected by means of purposive sampling and the data was collected by means of in depth interviews. Results: The results of the study revealed that the participants experienced a sense of loss of function after the brain injury, a fear of the future and loss of confidence in their worker roles. The participants also indicated that by means of adapting occupational routines and actively engaging in rehabilitation they developed confidence in their worker roles. Conclusion: Occupational therapists have to use a client centred holistic work integrative approach in order to successfully rehabilitate as well as facilitate the return to work process with people who have suffered a traumatic brain injury.Item The influence of divorce on the occupations of primary school going children in the Western Cape, South Africa: The perspective of the child and a relevant counsellor(Nova Publishers, 2014) Soeker, Mogammad ShaheedThere have been a number of studies over the years that propose that divorce could create turmoil in children’s lives. Many theorists agree that divorce is a stressful life transition needing adjustment for both the parent and/or child. This study determined how divorce influence childrens` participation in scholastic and leisure activities. The study was positioned in the qualitative research paradigm, specifically using focus groups and face to face interviews to collect data. The participants consisted of one male and three females (aged 8-12 years) and one adult female (key informant). Four themes emerged viz. 1) Listening but not hearing, 2) Adapting by doing, 3) Helping hands and 4) In the ideal world. The findings of the study indicate that children could benefit from family centred treatment programmes and should be involved in counceling post divorce. The barriers or challenges include feelings of sadness and hopelessness, inability to communicate and express feelings to parents and a change in social interaction and leisure activities. The facilitatory factors included having closer family networks, having a parental figure at home to assist with occupational needs, having positive peers support groups and counseling as a form of support.Item Mental health care users’ perceptions and experiences of a hospital-based vocational rehabilitation programme in a rural setting(University of the Western Cape, 2018) Msimango, Henry; Hess-April, Lucia; Soeker, Mogammad ShaheedReturn to work is a recognised goal of rehabilitation by the occupational therapy discipline that is addressed through vocational rehabilitation as a key feature of work practice. Programmes that fall under the umbrella of vocational rehabilitation include work preparation and training, work hardening and supported employment. In the case of mental health care users (MHCU), meaningful employment contributes to their sense of identity and the prevention of relapse. Vocational rehabilitation is thus important in preparing MHCU for self-employment or employment in the community after they are discharged from the hospital. There is however a dearth of literature pertaining to how vocational rehabilitation programmes particularly in a rural setting, is experienced by MHCU in South Africa. Therefore, the aim of this study was to explore the experiences and perceptions of MHCU regarding the usefulness of the vocational rehabilitation programme at a rural hospital in enhancing their worker roles. A qualitative research approach and exploratory descriptive research design was utilised. Purposive sampling was used to select participants for the study. Semi-structured interviews were conducted with ten MHCU participants as well as two focus groups. Semi-structured interviews were furthermore conducted with two key informants. All interviews and the focus group discussion were audio-recorded, transcribed verbatim and analysed through thematic data analysis. Strategies implemented to ensure trustworthiness included member checking and peer review. Ethics clearance was obtained from the UWC Research Committee and permission to conduct the study at the hospital was requested from the Mpumalanga Provincial Health Ethics Committee and hospital management. Three themes originated from the findings of this study. Theme one describes the barriers experienced by people with mental illness while returning to work. Theme two describes the programme factors that influence return to work in respect of work habits, work skills, and community re-integration. Theme three captures the value of participating in the work programme as experienced by the participants. The findings of the study is related to the Model of The Ecology of Human Performance (EHP) to show how the vocational rehabilitation programme effected the MHCU’s return to work. The findings show that the context as well as the person’s skills and abilities are very important factors in reintegrating the user back to work. The study illuminated recommendations for the further development of the vocational rehabilitation programme.Item Not STRAIGHT forward for homosexuals: A look at the lived experiences of homosexual men, living in Cape Town, with regard to their worker roles.(IOS, 2015) Soeker, Mogammad Shaheed; Bonn, G.L.; de Vos, Z.; Gognozi, T.; Pape, C.; Ribaudo, S.The South African constitution protects the rights of gays; however in the workplace gays experience discrimination and marginalization. As a result of marginalization they struggle to reach their potential regarding career development and the fulfilment of their worker role. Objective: The study explored the experiences and perceptions of gay males with regard to acquiring and maintaining their worker roles. The study is phenomenological and qualitative in design. Eleven of these men participated in two focus groups. One male participated in two in-depth interviews and one interview was conducted with a key informant. Three themes emerged: 1)Being boxed in, 2) The glass ceiling, 3) This is where I can wear my feather boa. The study findings clearly depicted the many barriers experienced by homosexual men and how this negatively impacts on their worker role. Minimal facilitatory factors exist, to assist gay males in regard to their worker role. It was found that homo-prejudice still exists in South Africa and its workplaces and has a negative impact not only on gay men's worker role but also their well-being. This significantly highlights the great need for occupational therapy intervention in the lives of these gay men, and their workplaces.Item Occupational therapy students’ perceptions of the challenges they experience in adapting to a psychiatric fieldwork placement.(International Research Journals, 2014) Soeker, Mogammad ShaheedBackground: Mental health in Occupational Therapy is experiencing a decline in preference for practice. Aim: This study explored occupational therapy students’ perceptions of the challenges they experience in psychiatric fieldwork. Methods: The researcher used a qualitative research design and collected data by means of focus groups with 8 final year occupational therapy students and 1 interview with a key informant. Findings: Five themes emerged: 1) Listening but not hearing, 2) Boxed thinking 3) Helping hands, 4) Losing the training wheels: Gaining independence and 5) An ideal psych programme. Implications: The barriers to learning identified in this study should be eliminated and the facilitators encouraged. The study highlighted that all theory be taught in the class lectures before the students enter their fieldwork placement. It was also recommended that multimedia sources such a video clips or recorded treatment sessions be used in formal lectures in order to familiarise students to the reality of psychiatry.Item Predictors of return to work among stroke survivors in south-west Nigeria(Sage Publications Inc., 2021) Soeker, Mogammad Shaheed; Olaoye, Olumide Ayoola; Rhoda, AntheaIntroduction: Stroke is acknowledged globally and among Nigerian rehabilitation researchers as a public health problem that leaves half of its survivors with significant neurological deficits and inability to re-establish pre-existing roles. Consequent to the dearth of country specific data on return to work and its determinants for stroke survivors in Nigeria, this study investigated the predictors of return to work among stroke survivors in south-west Nigeria. Method: Two hundred and ten stroke survivors from five tertiary health facilities in Osun state, Nigeria responded to a validated three-section questionnaire assessing return to work rates and its determinants after stroke in this study. Collected data were analysed using descriptive statistics and inferential statistic of chi-square, t-test and multiple logistic regression. Result: The mean age of the respondents was 52.90 ± 7.92 years. Over 60% of the respondents returned to work with about half of them in full time employment (32.9%). Majority of the respondents noted that travel to and from work (43.8%) and access at work (43.3%) had an impact on their ability to work. The symptoms of stroke (odds ratio (OR) = 0.87), the environment (OR = 0.83), body function impairments (OR = 0.86) as well as activity and participation problems (OR = 0.80) were the significant predictors of return to work. Hemiplegia or paresis of the non-dominant side of the body was associated with a higher chance of return to work (OR = 7.64). Conclusion: Body function impairments, activity and participation problems were independent predictors of return to work after stroke. Similarly, side of hemiplegia plays a prominent role in resumption of the worker role of stroke survivors in south-west Nigeria. © The Author(s) 2021.Item Returning individuals with mild to moderate brain injury back to work: A systematic client centered approach(InTech publishers, 2014) Soeker, Mogammad ShaheedAn increase in the numbers of individuals who sustained brain injuries due to motor vehicle accidents, trauma induced by violence and substance abuse, has resulted in more disabled individuals becoming non-productive members in society and inactive in the workplace. Research in the field of brain injury rehabilitation internationally is limited, with the majority of research focusing on the medical model of intervention. In the medical model, the disabled or injured individual is regarded as having problems that require medicalbiological intervention mainly, with little or no attention given to the difficult process of reintegrating the disabled individual back into society, for example, in resuming their worker roles. The medical approach may result in feelings of disempowerment on behalf of the disabled with regard to the rehabilitation process. The lack of success of current rehabilitation interventions could be seen as a result of an inability to generalize outcomes of rehabilitation in a clinical setting to the skills needed to return to work or re-integrate into the community.Item The value of occupational therapy intervention for the worker roles of forensic mental healthcare users in Cape Town, South Africa(IOS Press, 2021) Soeker, Mogammad Shaheed; Hare, Saabirah; Mall, Saadia; van der Berg, JeanBACKGROUND: There is a void in research that focuses on how OT programs facilitate self-esteem in forensic mental healthcare users' return-to-work in the open labor market, particularly in South Africa. OBJECTIVE: To explore the experiences and perceptions of forensic mental healthcare users, transitioning from an OT program to the open labor market. METHODS: Five individuals, diagnosed with Schizophrenia, and two key informants participated in this study that was positioned in a qualitative paradigm, with an exploratory and descriptive design. Semi-structured interviews were used to collect data. RESULTS: Theme one reflects on the challenges experienced by forensic mental healthcare users, when returning to work. Theme two describes various OT programs, as well as how they facilitate the reintegration of forensic mental healthcare users into the worker role. Theme three describes the positive support for forensic mental healthcare users, in the mental healthcare setting, workplace, and the greater community. Theme four describes how participation in occupation installs hope in forensic mental healthcare users, reinforcing the value of work, while positively influencing their work identity. CONCLUSION: The results revealed that stigma, residual symptoms, relapse due to substance abuse/mental health, and the lack of family support, were not be conducive to work skills development. However, the support of family and friends, workplace occupational therapy support, assembly line work, and therapeutic groups in an OT program, ensured positive workplace reintegration.Item Worker roles in the open labor market: The challenges faced by people with intellectual disabilities in the Western Cape, South Africa(IOS Press, 2021) Soeker, Mogammad Shaheed; Kaapitirapi, Philadelphia; Heyns, Megan; Shoko, Sofiso; Modise, WilliamBACKGROUND: Work holds great meaning and benefits beyond just monetary gain for people with intellectual disabilities. It gives these individuals the opportunity to engage in meaningful occupation. OBJECTIVE: The purpose of the study was to explore challenges that people with intellectual disabilities (PWID) experience when adapting to their worker roles in the open labor market. METHODS: The study used grounded theory as the research design. Five male participants and two key informants participated in the study. Two semi structured interviews were conducted with each one of the seven participants (five PWID and two key informants). RESULTS: Three core concepts emerged: 1) Unforeseen challenges of change; 2) A well-planned work preparation program enables success and 3) Crossing the bridge into the workplace: 'Do I belong here?' CONCLUSIONS: This indicated that with sufficient external support, PWID are able to gain a sense of social belonging and develop the necessary skills to cope with challenges that arise in the workplace when PWID transition from protective/sheltered workshops to the open labor market. The findings of the study also indicated that work preparation programs and supportive employment approaches helped PWID transition to the open labor market.
