Background: Medication adherence is a complex phenomenon amongst mental healthcare users on a global scale. The complexities of medication adherence may be due to factors such as demographic constraints, evolving technology and individual patient needs among others. Aim: The aim of this study was to investigate factors that influence medication adherence of mental health care users attending Community Health Centres in the Western Cape metropole region. Factors influencing adherence to medication were classified into three domains: medication related factors, patient related factors, and healthcare system related factors. Method: A descriptive survey, using a random sampling technique was used. The study utilised the Brief Medication Questionnaire (Svarstad, 1999) as a data collection tool. A survey design employing a descriptive approach and simple random sampling was utilised for the selection of the sample size of 102 (n=102). The sample included patients who have been diagnosed with a mental condition, receiving chronic medication from a Community Health Centre, who are a-psychotic during the mental status evaluation conducted by a mental health nurse prior to the data collection process and were aged 18 years and older. The data were analysed using Statistical Package for Social Sciences (SPSS), version 27. Findings: The overall scores for medication adherence in mental health care users at community health centres in the Western Cape Metropole Region were 83.60% for adherence and 16.40% for non-adherence. Of the total respondents (n=102), 77.45% (n=79) reported medication related factors, 21.57% (n=22) respondents presented with patient related factors, and 11.77% (n=12) respondents reported healthcare system related factors.
